We receive names and email addresses to match account and policy numbers. For payments through Online Services, we receive bank account information from our insureds. For first reports of injury (FROI) that are submitted through Online Services, we receive federal employer identification numbers, names, mailing addresses, phone numbers, social security numbers, birthdates, injury information and wage information. We receive written feedback. We receive this information directly from you when you provide it to us.
